Long Distance Authorization

This form is an alternative to filling out an application through our Telecommunications office in Fletcher 215.

Carefully fill out the form below and click the SUBMIT button. Every item must be completed. If the form is incomplete, you'll receive an error message and should click the BACK button of your browser to complete the form and try again. If all is well, you will be told your request has been submitted.

All bills will be sent to your dorm or apartment.  If you would prefer that your telephone bill be sent to your permanent address please select the check box and return 

Telephone Authorization Code Guidelines

All bills for long distance calls are due and payable by the 20th of each month. If a bill is not paid, the authorization code will be disconnected and a hold will be placed on the student’s records. The Student will not be allowed to register, receive grades, or graduate until all outstanding bills are paid. To restore service the student will pay the amount due, and then take the receipt to the Telephone Customer Service Office, who will reactivate the authorization code.

There is a $100.00 long distance charge limit. If calls exceed the limit the account will be suspended until the account is paid.  Students may establish a decline balance payment by making payments to their account. Note: This still leaves a remaining charge limit of $100.00 over the amount that has been applied to the decline balance before suspension of service. Decline balance may be carried over from one academic year to the Next. If at any time a student leaves the University, a refund can be processed for any remaining balance from our central accounting office in Knoxville. Students or parents will need to request the refund in writing.

It is the student’s responsibility to notify the records office of any address change or the Telephone Customer Service Office if a bill has not been received by the 5th of the month. DO NOT PUT CASH IN THE MAIL!

**WARNING TO STUDENTS**

You will be responsible for your authorization code number for the entire time you are enrolled at UTC. Therefore it is advisable not to loan your authorization code to anyone.  Report lost or stolen codes to Telephone Services at 425-4055.

After submitting your request, you will receive an email within 5 working days. This email will provide you with your long distance authorization code.
